At the outbreak of World War II in late summer 1939, the regiment took up positions on the Western Front as part of the 33rd Infantry Division. On November 1, 1940, the regiment was reorganized into the 115th Rifle Regiment when the 33rd Infantry Division was reorganized into the 15th Panzer Division.

The 115th Panzer Grenadier Regiment was established in Africa on July 5, 1942. The regiment was created by renaming the 115th Rifle Regiment. After its establishment, the regiment remained subordinate to the 15th Panzer Division. On October 30, 1942, the regiment transferred its 13th Company to the 155th Panzer Grenadier Regiment. This company was replaced by the 707th Heavy Infantry Gun Company (Sfl.). The regiment was destroyed in May 1943 in the Tunis area in Africa.

On July 15, 1943, the 115th Panzer Grenadier Regiment was re-established from the 2nd Grenadier Regiment, Sicily. The new regiment was subordinated to the 15th Panzer Grenadier Division after its establishment. The regiment was then deployed to Italy. After its deployment at Cassino, the regiment was disbanded in February 1944. Its personnel were incorporated into the 129th Panzer Grenadier Regiment.

In April 1944, the 115th Panzer Grenadier Regiment was re-established. The new regiment was created by renaming the former 129th Panzer Grenadier Regiment. The new regiment was also subordinated to the 15th Panzer Grenadier Division. It initially continued to serve in Italy. In September 1944, the regiment was relocated to the west.

The replacement of the first regiment is unknown. The 115th Panzer Grenadier Replacement Battalion was responsible for replacing the last two regiments.
